New sign ups please fill out the PAR-Q, client profile info and the three waivers below and bring them filled out to your first session (It only takes a few minutes). If you have any injuries, medical conditions, pain, etc., please fill out the health history questionnaire as well. As with the third document, you do not need to but consider filling out the fourth document (It will save time during our first few sessions)
These waivers are to be signed and brought in by first time sign ups for both class and in person training. Do not sweat if you forget or are unable to. We will take care of it in person. Class only clients do not have to fill out the consent.
Use these guides as references to help with nutrition, learning portion control, which foods you should eat more of and reduce within your diet, and a food journal you can use to write down what you eat. There is also a blank shopping list that has been pre-filled out and all you need to do is mark off next to what you need to pick up.
